whats the likely diagnosis? “Fatigue, general malaise.” A 55-year-old man presents to his PCP with his wife after a 4-hour history of “feeling bad.” This began suddenly while at home. He feels tired and does not want to do anything other than sit in his chair. He is winded quickly when he is moving around and feels like he has an upset stomach and believes it is indigestion. However, his wife notes he is not himself and she is worried about him. Upon further questioning, he does state he has pain and it is more over his chest than his abdomen. It does not radiate to his arm or neck. He has a hard time rating the pain, stating, “It just hurts.” His best description of the pain is that it is dull. Sitting in his recliner seems to ease his pain. He is on hydrocodone/acetaminophen 10/325 mg for chronic lower-back pain, and this has not relieved his current symptoms. He has not wanted to eat anything today due to fatigue. His wife also notes he appears sweaty even though he has not done much all day. The patient admits he has not been faithfully checking his blood pressure as he was asked to do on a prior visit. When he does check, which is approximately once a week, it is in the range of 140 to 150 for systolic; he does not know his diastolic blood pressure. His last cholesterol level was checked 4 months ago, and his LDL was mildly elevated at 140; total cholesterol, HDL and triglycerides were within normal limits.
