Physical Exam Techniques with Expected/ abnormal findings for Lobar Pneumonia a) Auscultation of Breath sounds Technique: Using the diaphragm of the stethoscope, auscultate anterior, posterior, lateral and posterior lung field and compare side to side Expected abnormal findings: Bronchial breath sounds Crackle (fine or coarse) due to fluid being present in the alveoli Increased vocal resonance such as: Bronchophony is a voice sound assessment In this procedure we ask the patient to repeat “99” while listening with the stethoscope over the chest wallNormal finding is normal voice transmission is soft, muffled and indistinct, you can hear sound through the stethoscope but cannot distinguish exactly what is being saidAbnormal finding is disease that increase lung density enhances transmission of voice sounds, when auscultating, you clearly hear the words “99”. The words are more distinct than normal and sound close to your ear. (Jarvis et al., 2019) Egophony: Auscultate the chest while the patient phonates a long “eeeee” sound.Normal finding is you should hear “eeee” through your stethoscope.Abnormal finding is over area of consolidation or compression, the spoken “eeee” sounds like a bleating, long “aaaaa” (Jarvis et al., 2019) Whispered Pectoriloquy In this assessment, patient is asked to whisper a phrase such as “one-two-three” as you auscultate.Normally finding voice transmission is faint, muffled, and almost inaudible.Abnormal finding is with only small amount of consolidation, the whispered voice is transmitted very clearly and distinctly, although it is still somewhat faint, it sounds as if the patient is whispering “one-two-three” right into the stethoscope. (Jarvis et al., 2019) B) Percussion of the thorax Technique Percuss intercostal spaces over lung fields, comparing left to the right Expected abnormal findings in lobar pneumonia Dullness to percussion over areas of consolidation This provides us with information that fluid or solid tissue replacing air in the alveoli, consistent with pneumonia
