Preoperative Diagnosis: Urinary retention. Postoperative diagnosis: Same. Operation Performed: Cystoscopy transurethral prostatectomy and bladder neck incision. Anesthesia: General. Blood Loss: 100 CC. Drain In Place: A 22-French 3-way Foley catheter with 40 cc in balloon. Procedure: With the patient in the dorsal lithotomy position under general anesthesia, he was prepped and draped in a routine sterile manner for a cystoscopy. A 23.5 cystoscope was introduced into the bladder under direct vision. There was noted to be a trabeculated bladder with catheter reaction and an obstructing 2.5-cm prostate. At that point, the scope was removed and the 24-French Teflon resectoscope sheath was placed after lubricating it with K-Y jelly and Vaseline. Using a cutting current of 170 and a coagulation current of 70, the procedure was performed. A groove was cut from the bladder neck to the verumontanum. The left lobe was then resected posteriorly to anteriorly, and a similar resection was performed on the right. Additional tissue was removed circumferentially from the apex. Hemostasis was accomplished. The chips were irrigated out of the bladder. There was still tightness of the bladder neck, so bladder neck incisions were made with a Collings knife, which opened up the bladder neck. The bladder was filled, the scope removed, and a 22-French 3-way Foley catheter was inserted, placed on moderate traction on the left leg after inflating it to 40 cc. The catheter irrigated well, and the patient was taken to recovery in good condition. ICD-10-CM Code: CPT Code:
